Welcome to Camp Shalom 2011! We know there is a lot of information
to look at regarding Camp Shalom, but please take the time to
download the forms. It is a
state requirement that all forms be completed prior to
the start of camp. We are asking that you turn in completed
forms no later than May 12th. Don’t forget
we also need the Florida Certification of Immunization and Student
Examination Form from your child’s pediatrician (blue and yellow forms). Please
don’t hesitate to contact the Camp Office (561-259-3000)

Letter
|
• This is a letter from Yale David, Camp Director. Please take the time
to read this letter.

Camper
Information |
• This gives the camp a little more insight
on your camper.
|
|
|
Health
History and
Pick Up Authorization |
• This form provides camp with the necessary
health information regarding your camper, gives camp permission
to transfer your child in the unlikely event of an emergency,
and informs Camp Shalom of the people who have authorization
to pick up your child from camp each day.

Picture
Permission Form |
• This form does or does not give the JCC permission
to take and/or use photos of your child.
|
|
|
Group
Request Form |
• This gives you the opportunity to request
other children to be grouped with your child. It is necessary
for us to have this form no later than May 14th to ensure your group request.

Authorization
Form |
• This form authorizes the JCC for several different
items we will use throughout the summer. Please be sure
to complete this form.
|

|
Carpool
Information |
• Please fill out this form to let us know how
your child is getting home daily and if they have a sibling
and/or you are carpooling with another family on a regular
basis.

|
Medication
Authorization Form |
• This form needs to be filled out by the parent
and signed by the doctor if medication of any type is
needed to be distributed during the camp day.

Special
Needs Intake Form |
• For those children in Tikvah, this form in
mandatory.
|
|
|
Pump
It Up Waiver |
• It is mandatory for all Tikvah campers to complete
this form
in order to participate in the field trip to Pump It Up.
