Welcome to Camp Shalom Yeladim 2015 at the Edward & Thelma Glantz Early Childhood Learning Center. Please take a moment to download the parent packet. This packet includes enrollment forms, supply list and other important information. It is a state requirement that all forms must be completed and submitted prior to the start of camp. Forms must be completed and returned to the preschool office no later than May 22th along with the Florida Certification of Immunization and the Student Examination Form. The medical forms are obtained from your child’s pediatrician’s office. If you have any questions, please contact the preschool office at 561-732-7620. Please be aware that children cannot attend camp unless all forms are submitted to the preschool office.

We are delighted that you and your child have chosen the Mandel JCC Camp Shalom. We consider ourselves to be your partner and feel that in working together we can ensure that your child has an incredible camp experience.
Please find our Camp Shalom Family Guide 2015 below, It is filled with information to answer any and all questions you have. Should you need additional information please feel free to call our camp office at 561-259-3000 and speak with Vicki. There are also links to the various forms that need your attention. Don’t forget to obtain the Florida blue and yellow health forms from your child’s doctor. ALL FORMS are due back to the camp office by FRIDAY, MAY 29th.
Camp Lunch Order Form- Don’t want to pack a lunch, no problem! Gourmet Deli House is catering lunch every day this summer!! Download this form to find out more information and to register!
Circus Letter- Information for those attending Circus Camp.
Discipline policy- To be signed by both camper and parent.
Florida Minor Release- This form is required by the state of Florida to have on file for every minor who participates in a recreation program throughout the state. It is mandatory that the Camp Office receives this form signed prior to the start of camp. Please note: only one parent or guardian needs to sign this form.
Group Request Form- We will do our best to honor any requests made, but it’s not a guarantee.
Medication Authorization Form- If your camper takes medicine during the camp day this form must be completed.
Health Form and Immunization Records- Florida Blue and Yellow forms provided by Pediatrician. ALL campers must have these forms at the camp office in order to attend camp.
Pick-Up Authorization Form- This is provided if you would like to update your authorized people from initial registration.
Unit Calendar- Can be downloaded under “Typical Day” explanation for each unit on our Camp Website.
