Camp Shalom 2012 Parent Resources

Welcome to Camp Shalom 2012! We know there is a lot of information to look at regarding Camp Shalom, but please take the time to download and complete the necessary forms.

It is a state requirement that all forms must be completed prior to the start of camp. We are asking that you turn in completed forms no later than May 18th. Don’t forget we also need the Florida Certification of Immunization and Student Examination Form from your child’s pediatrician.

Please don’t hesitate to contact the Camp Office (561-259-3000) if you have any questions, or would prefer a mailed copy of these forms.

Welcome Letter	This is a letter from Rachel Fox, Camp Director. Please take the time to read this letter.
Circus Camp Letter	This provides information regarding Circus Camp. For those participating in Circus, it is necessary to read this letter.
Chugim Form	New this year for general campers! Chugim is the elective period. The chugim are assigned on first come basis, so please fill this form out and return it to the camp office as soon as possible. (not applicable for Sports Camp or Tikvah)
Camp Lunch Form	Fro Yo on Thursdays and Chipotle on Fridays, what could be better? Download this form to find out more information and to register!
Group Request Form	Use this form to communicate the group requests for your camper.
Medication Authorization	If your camper takes medicine during the camp day this form must be completed. If your camper takes prescription medication during the day it must be signed by your child’s physician.
Florida Minor Release Form	This form is required by the state of Florida to have on file for every minor who participates in a recreation program throughout the state. It is mandatory that the Camp Office receives this form signed prior to the start of camp. Please note: only one parent or guardian needs to sign this form.
Camp Shalom Play	All campers are encouraged to participate in this year’s play, “A Hatful of Seuss” This will provide you with more information including the registration.
Special Needs Intake Form	For those children with Special needs (Tikvah and Inclusion), this form is mandatory.
Backpack Basics	This form informs you of what to make sure your child needs to have each day, and what to leave at home.
Carpool Flyer	Includes procedures for drop off and pick up at camp.
General Information	This flyer provides you with general Camp Shalom information that you won’t find anywhere else in this packet.
Chugim Camp	Camp doesn’t end August 3rd! Join us for our first ever Chugim Camp!
Post Camp - available soon	There is still more fun to be had! Check out our option the week before school starts.
Nitzan Calendar	Calendar for those in the kindergarten unit.
K’Tanim Calendar	Calendar for those in the 1st grade unit.
Kesher Calendar	Calendar for those in the 2nd/3rd grade unit.
Kochavim Calendar	Calendar for those in the 4th-6th grade unit.

Surf Camp Waiver	All Surf Camp Campers must have this waiver complete in order to participate in Surf Camp.
Horseback Riding Camp Waiver	All Horseback Riding Camp Campers must have this waiver complete in order to participate in Horseback Riding Camp.
Worlds Largest Swim Lesson Waiver	All campers who attend Camp Shalom week 1 will be in the Guinness Book of World Records for participating in the Worlds Largest Swim Lesson! The waiver must be completed prior to the start of camp.
Kesher Waiver	Kesher Campers attending camp week 2 must have this complete to go on the field trip.
Tikvah Waivers	Tikvah campers must have these waivers completed to participate in some of the field trips.
Kochavim Waivers	Kochavim campers must have these waivers completed to participate in some of the field trips.
