Welcome to Camp Shalom 2015! We know there is a lot of information to look at regarding Camp Shalom, but please take the time to download the forms and complete the necessary forms. 

It is a state requirement that all forms must be completed prior to the start of camp. We are asking that you turn in completed forms no later than May 20th. Don’t forget we also need the Florida Certification of Immunization and Student Examination Form from your child’s pediatrician. Please don’t hesitate to contact the Camp Office 561-712-5227 if you have any questions, or would prefer a mailed copy of these forms. 

Welcome Letter- This is a letter from Rachel Fox, Camp Director.  Please take the time to read this letter.

Circus Camp Letter- This provides information regarding Circus Camp.  For those participating in Circus, it is necessary to read this letter. 

Florida Minor Release Form- This form is required by the state of Florida to have on file for every minor who participates in a recreation program throughout the state.  It is mandatory that the Camp Office receives this form signed prior to the start of camp. Please note: only one parent or guardian needs to sign this form. 

Camp Lunch Form- Don’t want to pack a lunch, no problem!  Gourmet Deli House is catering lunch every day this summer!! Download this form to find out more information and to register! 

Chugim Form- Chugim is the elective period.  This year the campers are going to pick their own chugim, but download this form for more information on this period.

Group Request Form- Use this form to communicate the group requests for your camper. 

Medication Authorization- If your camper takes medicine during the camp day this form must be completed.  If your camper takes prescription medication during the day it must be signed by your child’s physician. 

Special Needs Intake Form- For those children with special needs (Tikvah and Inclusion), this form is mandatory.

Backpack Basics- This form informs you of what to make sure your child needs to have each day, and what to leave at home. 

Nut Free Zone- Please read this flyer to find out about the new Camp Shalom Nut policy. 

Carpool Procedures- Includes procedures for drop off and pick up at camp.

General Information- This flyer provides you with general Camp Shalom information that you won’t find anywhere else in this packet. 

Surf Camp Waiver- All Surf Camp Campers must have this waiver complete in order to participate in Surf Camp.

Sailing Camp Waiver- All Sailing Camp Campers must have this waiver complete in order to participate in Sailing Camp.

Kindergarten Waivers- All campers entering kindergarten who attend camp week 1 must have this waiver completed in order to participate in the Zip into Camp Special Event.

1st Grade Waivers- K’Tanim campers who are attending camp week 1 and week 5 must have this complete to go on the trip. This includes sports and general camp. 

2nd/3rd grade Waivers- Kesher campers must have these waivers completed to participate in some of the field trips. This includes sports and general camp. 

4th/5th grade Waivers- Kochavim campers must have these waivers completed to participate in some of the field trips. This includes sports and general camp. 

Giborim Waivers- Giborim campers must have these waivers completed to participate in some of the field trips.

Tikvah Waivers- Tikvah campers must have these waivers completed to participate in some of the field trips.
